With the institution of the new PIAA Medical Clearance form (CIPPE), there are new procedures to follow for having medical evaluations done. 

One CIPPE form is valid for the entire school year.  However, after playing in a sport season, the student's parent must complete and submit Section 5 of the CIPPE for each and every subsequent sport season.  If a student-athlete is injured and is required to miss any scheduled contest, then Section 6 of the CIPPE must be completed and submitted by a Certified Medical Examiner for the subsequent sport season. 

The FREE Winter Sports Clinic for Hazleton Area athletes is scheduled for Saturday, October 25.  This clinic is available for any student who has not already submitted a CIPPE for the year.  Students should download the CIPPE, have a parent complete sections 1 through 4, and submit the form to Mr. Young by Friday, October 17 if they plan to attend the clinic.  Students not planning on attending the clinic will need the form completed by a Certified Medical Examiner and submitted to Mr. Young before they may attend any winter sports practice.
